you have visited the practice for a face-to-face service in the last 12 months
Please note: In the event that you are not eligible to receive a telehealth appointment, the doctor is unfortunately unable to proceed with the booking. It is important to ensure you check your eligibility prior to booking to avoid missing an appointment)
